Family Medical Associates of Raleigh (FMAR), a North Carolina primary care provider, has reported a data breach that may have exposed personal and health information.
According to a notice posted to its website, FMAR became aware of suspicious activity within its computer systems on May 7, 2026. Following an investigation with data security and privacy professionals, the healthcare provider determined that an unauthorized actor gained access to its computer systems and downloaded certain information between April 18 and April 20 of the same year.
Personal information that was potentially compromised in the FMAR data breach includes names, addresses, biometric and genetic data, contact information, dates of birth, demographic information, financial account numbers (without access information), Social Security numbers and taxpayer identification numbers.
The incident may have also affected health information—namely, dates of treatment, service and discharge; lab results; medical history, such as treatments, diagnoses and procedures; medical record numbers; physical or mental conditions; prescription or medication information; Medicare or Medicaid IDs; treatment cost information; health insurance group numbers and policy numbers; health plan beneficiaries and beneficiary numbers; and health plan numbers. The degree of personal and health information exposed in the incident varied by individual.
The Family Medical Associates of Raleigh data breach was also reported to the Vermont Attorney General’s Office on October 2, 2026.
